    About Us

    When a person is diagnosed with a life limiting illness, it is their unpaid caregivers, family-spouse-friend, that are the everyday champions who graciously support them through this life transition

     

    Caregiver Transition Supports (CTS) is a palliative-focused, resource-rich service that provides relief to unpaid caregivers. We do this by accessing resources and coordinating quality-of-life supports including end-of-life planning through our trusted partnerships with non-profit and for-profit providers. 

     

    Our goal is to provide relief to caregivers—spouses, family members, and friends—to regain balance, reduce stress, and find moments of peace while continuing to provide care during one of life’s most important transitions.

    Why CTS?

    In Alberta, a 2018 survey identified nearly one million unpaid caregivers
    approximately one in four spouses, family members, or friends—who
    provide a minimum of nine hours per week supporting someone in need
    (*Caregivers Alberta, 2018). Today, that number is estimated to exceed one
    million Albertans.


    For many caregivers, where to start managing responsibilities can become emotionally and
    physically overwhelming as they navigate the complexities of a life-limiting
    illness affecting the person in their care.


    Caregiver Transition Supports (CTS) is a resource-rich
    hub that
    provides compassionate relief to unpaid caregivers
    . We do this
    by partnering with non-profit and for profit partners to access resources for supporting life transitions, coordinating daily care logistics, and offering conversation and companionship along the caregiving journey.


    Our services are thoughtfully designed to complement—not replace—the
    essential care provided by families, healthcare professionals, and
    community service providers.

    We can assist caregivers and their care recipients connect with a registered  End-of-Life-Doulas for their planning.

    Our team assists new or overwhelmed unpaid caregivers access resources by working with non-profit and for profit partners that ease the caregiving experience.

    We provide key information on hospice and can assist coordinating self-referrals for hospice admissions.

    What we Do-Our TLC Approach

    Caregiver Transition Supports (CTS) supports unpaid caregivers  through our TLC approach:

    T - Transitions: We help coordinate care transitions by assisting families navigate palliative and chronic care needs, planning for end-of-life , and assisting with self-referrals for hospice admission.

    L - Logistics: We help coordinate the complex care and daily needs that arise during serious illness, supporting comfort, dignity, and quality of life while helping caregivers prevent burnout and restore balance in their lives.

    C - Conversation & Companionship: We provide compassionate companionship through phone or online connection, offering meaningful conversation and supportive presence for those in their care while giving caregivers a moment of rest and reassurance.

    Qualified, Trained, and Trusted Support

    At Caregiver Transition Supports (CTS), our team and associates hold baseline, documented training aligned with recognized palliative care and caregiver support standards. We maintain active WCB coverage to ensure safe and responsible service delivery. CTS is committed to continuous professional development so we remain current in best practices and uphold the progressive standards of the palliative sector.

    Examples of our Help 

    Transitions: When the David family needed help navigating end-of-life planning and a self-referral for hospice for their mother, Ruth, CTS helped coordinate the details and guide them through each step of the journey.

     

    Logistics: When coordinating care for Matthew’s advanced chronic heart condition became overwhelming for Elizabeth, CTS helped her organize the details so she and her husband could spend more peaceful time together. 

    Conversation & Companionship: When the days felt long and emotional for John, CTS offered conversation and companionship for both him and his mother Hannah, creating moments of connection and comfort.
